Reason : Breaking the Terms of Use

Raging, Cussing, being Rude, just to name a few. just read his post in the evidence and you will know what i mean. I have no who he is, or what his problem might be, but id say he needs some time to calm down.

You couldn't find one because it doesn't exist. There is no such thing as a forum ban request. Just report his ToU infringing posts to moderators and they will deal with him accordingly.
